This agreement is between Celiac Coach ("Coach") and you ("Client") for functional health coaching services. This is a placeholder document — please review with your legal counsel before publishing.
Scope of coaching
The Coach provides education, guidance, and accountability. Coaching is not medical, mental health, or nutrition counseling and does not diagnose or treat disease.
Client responsibilities
- Provide accurate and complete information during intake.
- Consult your healthcare provider about medical concerns.
- Show up on time and prepared for sessions.
- Communicate openly about what is and isn't working.
Confidentiality
Information you share is treated as confidential and will not be disclosed without your consent, except as required by law.
Cancellations
Provide at least 24 hours' notice to reschedule or cancel a session. Late cancellations may be subject to the full session fee.
Payment
Session fees are due at the time of booking through Stripe. No refunds for completed sessions.
